Eligibility for this service

Community Connect Transport-1Community Connect Transport services are funded for either older people who need assistance with daily living to keep living independently at home and in their community or younger people who have a permanent functional disability, live in the community and are at risk of premature or inappropriate admission to residential care. Carers of these customers will also benefit from our transport services.

People over 65 years

For people over 65 years of age, and for Aboriginal and Torres Strait Islanders over 50 years of age the Australian Government provides help through My Aged Care, which helps you or a carer find the information about aged care services. This can be as simple as calling the My Aged Care contact centre on 1800 200 422 or visit My Aged Care website. If you need help or have any questions please call Community Connect Transport Services on 9490 9500 When you call My Aged Care, the contact centre staff will ask questions to help understand your needs. My Aged Care provides you with information about:

  • Different types of aged care services
  • Eligibility for services
  • How aged care needs are assessed
  • How to find local services to meet your needs (you are able to select your own service provider)Please advice the My Aged Care team that you wish to use Community Connect Transport Services as your preferred transport provider, if this is the case.
  • Costs of aged care services, including free estimators.

To help you find the right services, the contact centre will ask for your consent to create a personalised client record. A client record holds up-to-date information on your needs and any services you receive. The client record will reduce the need for you to retell your story to the contact centre, assessors and service providers.

People under 65 years

For people under 65 years of age, and for Aboriginal and Torres Strait Islanders under 50 years of age the NSW Government provides help through the National Disability Insurance Scheme(NDIS). The NDIS means a change in the way and how people with a disability access supports. It is about exercising choice and control over those supports – what they are and who will provide them.

The NDIS will be rolled out in Northern Sydney between 1 July 2016 and 30 June 2017.
