Print the form to complete your details and either give your completed form to your Driver on your next trip with Community Connect Transport, or scan and email the form to: info@communityconnect.org.au or call the Office on 9490 9500 to book.

Reservations and Pre-payments will be required by June 7. 

Lunch is a 4 Course Set Menu and the cost is $75.00 per person, plus your transport fee.

Please note no refunds will be given for any cancellations made after July 3.

This Social Outing is exclusive to Community Connect Transport Clients and is for independent travellers only.

If you are not a Registered Client of Community Connect Transport, but would like to join us for Christmas in July, please contact myagedcare on: 1800 200 422 to check your eligibility for travel with us.
