Overview
Community Connect Transport provides safe and reliable transport services to eligible clients, who live independently and are unable to access mainstream public transport in the Local Government Areas of Lane Cove, Mosman, North Sydney and Willoughby.
Community Connect Transport services are funded for older people who need assistance with daily living to keep living independently at home and in their community, or younger people who have a functional disability and who maybe at risk of premature or inappropriate admission to residential care.
The Community Connect Transport services office is open Monday to Friday, 8.00am to 4.00pm and our wonderful drivers are available to transport residents from 7.30am to 5.30pm Monday to Friday.
We Offer Transport for Medical Appointments
Safe, reliable and flexible “point-to-point” transport services for medical, hospital, specialist, and rehab appointments, in either cars or buses depending on our client’s needs. Please note we do not provide hospital in-patient discharge transport. We do transport people to hospital but not return if they have undertaken procedures.
Our vehicles are selected for ease of access, and many are wheelchair accessible enabling passengers with various levels of mobility a choice of transport. Our clients include older people, people with disabilities, people from culturally and linguistically diverse backgrounds, children, youth, seniors, Aboriginal and Torres Strait Islanders and Carers.
Our weekly Shopping Services provide clients with reliable, regular access to the shops. Clients can book a permanent pick up or travel occasionally. Carers can accompany clients at no extra cost. A limit of two shopping bags per person applies. Bookings are essential.
Community Connect Transport has a program of social outings and events each month. These outings provide our clients with a wonderful opportunity to get out and about and explore new locations. They can join us for a delicious lunch, visit an art gallery or museum to view the latest exhibitions, take a shopping excursion, explore flower, and plant nurseries, enjoy coffee and a snack in cosy cafes and more.
Clients have the chance to meet new people, build friendships and expand their social network. Our drivers and social volunteers on each trip are always ready to listen and can provide assistance where needed. The cost of the outings varies, depending on the destination and any meals or applicable entry fees are not included, unless stated in the program. See our Social Outings & Events calendar here.
Full-cost service can be provided to clients on Home Care Packages and often through negotiation with their HCP provider. We provide transport services on a situation basis for the client. Once a referral is received from MyAgedCare, we will send out an information pack and your client will be required to complete a Community Connect Transport registration form. Please direct clients to our website communityconnect.org.au should they wish to complete the New Client Registration form online. Once registered, clients can make transport bookings either by phone on 9490 9500 or through the website. We ask for bookings to be made at least three working days in advance, otherwise they may be added to a standby list.

Home Care Package Providers
If you are looking for transport for your client on a Home Care Package (HCP), Community Connect Transport can assist with transport to everyday appointments and shopping centres and will happily take a Carer for no extra charge. Our Social Outings are very popular with clients on a variety of programs including Home Care Packages. Simply register your client, request a quote, and provide us with your invoicing details and any contracts you require us to sign.

Driver Requirements
All our drivers are fully qualified, have undergone a medical, criminal record check and hold a first aid certificate. These are minimum requirements and along with many modules they are required to pass these requirements, are re-tested every three years and some every 12 months. Our drivers must comply with all the Transport for NSW qualification requirements and are regularly re-tested in various disciplines. This standard is at a higher level than many other transport providers. Safety is our number one priority for all clients.
How do I book Community Connect Transport?
Phone bookings can be made with our Customer Service Team on 9490 9500. If the phone is unattended please leave a message.
To assist our Co-ordinator whilst making a booking please have the appointment time, full address of destination, doctors name/clinic number and the surgery contact telephone number on hand. Also please advise how long you anticipate the appointment will be.
To help avoid disappointment is it best to book more than 3 working days in advance whenever possible.
